 Perryburg Schools is now accepting

online pre-registrations for 2017-18 Grades K-12 Students

for Perrysburg School District Residents using InfoSnap

(Student needs to be five on or before August 1, 2017 to attend kindergarten)

 Please note: Parents/Guardians of students currently attending Perrysburg’s preschool program at Union Preschool do not need to pre-register if their student is transitioning to kindergarten.  However, current Union Preschool Parents/Guardians will be asked to verify their current information in our system. Additional information will be provided from our Preschool staff about the verification process and the required student assessments.
Parents/Guardians of the new students for 2017-18 should select the following link to pre-register:  New Student Enrollment for 2017-18
After parents/guardians complete the New Student Enrollment for 2017-18, they will be asked to Schedule an enrollment appointment, which will be held at Central Office. 

Parents/Guardians will schedule an enrollment appointment from the submission confirmation page of the enrollment form, using the designated link provided.

Note: Parents/Guardians need to complete the New Student Enrollment for 2017-18 and schedule an appointment time to be considered pre-registered. 

Parents/Guardians should plan to bring to the scheduled appointment the required documents (certified original birth certificate, photo ID as the parent/guardian, proof of residency - see “New Student Enrollment” tab for detailed description by selecting the link), immunization record, and complete court file-stamped custody papers (if applicable) if they did not upload the documents into the InfoSnap program.

For New 9th Grade High School Students ONLY:

The student will need to take the STAR Math and Reading Test. This is a test for enrollment that is done on the computer and will take about 45 minutes. The STAR Reading and Math Test results are to aid the high school in your child's proper class placement to ensure their success at Perrysburg.  This test may be done at the time of the enrollment appointment or scheduled at a later date if needed.
The following items need to be presented at the scheduled registration appointment if they were not upload into InfoSnap. Copies will be made during that time. Your child is NOT considered “enrolled” until ALL items are received.
  1. Photo I.D. of Custodial Parent/Guardian must be presented when registeringAlso, any paperwork showing a parental name change (i.e. divorce/re-marriage) must be presented.
  2. Proof of Residency.
  3. Certified Original Birth Certificate.
  4. Immunization Record (Immunization records from the student’s birth to the present must be submitted. For kindergarten students, the physical form MUST be submitted prior to the first day of attendance.)  For Preschool students, the Preschool Medical Assessment MUST be submitted prior to the first day of attendance.)
  5. Court file-stamped Custody Papers, if applicable to your child.
  6. Most recent grade card.
  7. Current transcript or most recent class schedule (High School Only).
  8. Special Education Records (IEP/ETR), if applicable.
  9. Written Education Plan (WEP), if applicable.
  10. 504 records, (evaluation/plan) if applicable.
  11. Parent Request and Authorization to Administer a Prescribed/Over the Counter Medication or Drug if your child will need to receive medication during school hours. 

In order to accommodate your registration quickly, we MUST comply with the legal requirements of the following:

  • Federal/State Law - “Missing Child Act”
  • State Law ORC 3113.64 – Legal Custody
  • State Law ORC 3313.64 - Legal Residency
  • Perrysburg Board of Education Policy Guidelines #5111
